I have just released z3c.relationfield 0.4.1 which should fix this issue.

I'll note that I had to put in a "return ''" at the end of the add 
handler to prevent another error in z3c.relationfieldui from being 
triggered that I haven't quite figured out yet. The return '' at the end 
of the handler will make sure that no rendering of the form takes place 
that is immediately redirected anyway. Instead the content of the page 
will simply contain ''.

It'd be a nice bugfix to Grok to try to make this happen automatically 
in case of redirects, as I can't really think of a good reason not to 
let this happen and this is a bit of a pitfall.
