Hmm, this works for me on a customized search form:

  <td>
   <select name="Subject:list" multiple size="4">
    <dtml-in expr="portal_catalog.uniqueValuesFor( 'Subject' )">
     <option value="&dtml-sequence-item;"> &dtml-sequence-item; </option>

 </tr>
