<dtml-var foo fmt="structured-text"> starts the "heading level" at 3;
likely it does this as a compromise, to make embedding STX in a page
more sensible (defaulting to H1 is not perfect, either).

We need to fix Document.py to work directly with STX;  this will allow
us both to change the policy, and to extend the recognized markup (e.g.,
to allow inline image references).

Please file it as a Tracker issue, so we don't forget about it:
