When you just customize using the ZMI you do not need to touch the  
skins at all. The order of skin folders in a skin can be seen in the  
portal_skins tool. Click on the "Properties" tab and you will see the  
order for each skin.

Everyone does that differently, the spectrum reaches from hardcoding  
navigation elements in the template all the way to doing catalog  
requests to pick out what to show. I don't know your requirements, so  
I can't really comment.
