Hello Andrew,
Did you specify enctype="multipart/form-data" in your FORM tag?
Here is an example from a ZClass I use:
<FORM action="PDFFileClass_add" enctype="multipart/form-data"
method="post">
...
<INPUT type=file name="file" size="30">
...
</FORM>
