We use UML for all of our modeling (along with a modified Unified Process
for our customer projects).  Currently, our modeling tool of choice is
Object Domain, but I have a distinct feeling this will change.  We have also
used Rational Rose for some projects.

Code is all maintained in CVS with links back to Use Cases that are being
implemented (we number use cases for reference).  Documentation is in a
combination of structured text for internal, and Microsoft Word for
external.
