There have been several reports of weirdness involving reindex_object
in 2.1.6, and no fix that I know of.  A reported workaround is
to call reindex_object by doing a redirect to it at the end of your
instance(Edit|Add) method, but I'm sticking with 2.1.4 for now
because of this problem.
