Scavenging the mail folder uncovered Paolo Comitini's letter:

the Translator does not descend a folder hierarchy. trnslated
objects should be placed in a flat hierarchy inside the language
folders. do as follows:

http://www.website.com/FH1/Translator
http://www.website.com/FH1/Translator/en/index_html
http://www.website.com/FH1/Translator/de/index_html

then 

<dtml-with FH1>
<dtml-lvar index_thml>
</dtml-with>

will work.
