Hmm.  It looks as though the wrong Apache VirtualHost is getting 
triggered, except that your Rewrite log entries look correct.

What do you get if you create a DTML Method containing the following and 
then view it through the virtual host?

<dtml-var URL>
<dtml-var PATH_INFO>
