Right.  That's what structured text considers a "paragraph".

It seems that to generate a button I need
blank lines either side of the text, and a
carriage return inside the text.

That's a toggle button. When it says "COM off" it's asking you if you
want to turn comments off.

I suspect you've only put one line of text in the body of the
document,
and thus BackTalk thinks its a header (which don't have comment
buttons).

Try adding more text, specifically a multiline "paragraph".
