[Zope] FW: Formulator not working

Sorry if this did make it yesterday, but I never received a copy so I'm
posting again...


Hi all,
 
I've installed Formulator in my zope setup, and I can see it in control
panel -> products.

When I try and add a formulator form, I get the following error:
