Try something like:

      <select name = dstring size = 20 >
             <dtml-in  "execsearch(REQUEST)"  >
                 <option value="<dtml-var name="sequence-item"><dtml-var
name="sequence-item"></option>
             </dtml-in>
             <option value="all">All</option>
      </select>


Then, in your form processing routine check for dstring = 'all'
