Usually, you should be able to use trivial URL references (of
the form "filename").

When you use the implicit "index_html" URL, this depends
on a correct "base" tag.
Due to bugs (recently, I met such a bug in CMF/Plone), the
"base" tag generation is sometimes "forgotten".
