When I try this with a file that is named with a lowercase .htm extension, no <base> is generated, and the browser fetches all relative <img> tags just fine. Apparently, you get an object with an "index_html" method that is not "None". In this case, ZPublisher calls the "index_html", adds an "base" tag and appends a '/' to the URL as it would be necessary for a folder.
