[Zope-PAS] Zope2 challenge patch

I've attached a patch against current CVS which implements Jim's basic idea.
It includes a patch to the cookie authentication so that it either redirects
(if the first challenger called), or includes the login form in the response
body.

As has always been the case, the semantics for setting the response code and
body are not clear - but they do seem to work well enough for all the
examples we have today.

My testing indicates that HTTP, NTLM and cookie auth all work together as
well as they possibly can.
