Rajeev,

You did not mention the mod_proxy module in your configuration.

Either load the module or compile apache with --enable-module=proxy

Additional configurations are:

Listen 80
ProxyVia On

Do you have ErrorLog, TransferLog, and RewriteLog specified? They should give you hints what goes wrong.
